Health Appraisal- Please print and fill out personal information and Section I & then take it to your child's physician or medical professional to complete Sections II-IV. We must have a completed Health Appraisal within 30 days of your child's first day of attendance.
If your child has an allergy, a Medical Action Plan (MAP) is required to be reviewed by the RCS District Nurse prior to starting class. More information can be found on the Health & Wellness page.
