Medical Action Plan (MAP)
2022-2023 School Year
Dear Parent(s)/Guardian(s),
Rochester Community Schools (RCS) is committed to ensuring the health and safety of all students! Updated Medical Action Plans (MAPs), are required at the start of every school year and expire on the last day of school instruction. The MAP provides RCS with required medical direction from your primary, licensed health care provider and allows staff to provide services, such as, medication administration, medical interventions and/or medical procedures. Safe and legal interventions (without prescriber orders), will be provided and include: soap, water, ice, comfort, and adhesive bandages.
AFTER mid-August, 2022, forms can be submitted to the school that the student will be attending for the 2022-2023 school year. If you are new to the district and your student has a medical condition that is dependent on staff support, please contact the school directly, prior to school start.

Medication Administration
Please complete this form for dispensing medication for conditions not listed below; such as asthma, diabetes, seizure, and severe allergies.
Medications will not be administered if expired, or for the treatment of symptoms, such as fever, respiratory/gastrointestinal illness, etc., unless there is a medical action plan on file to support a chronic medical condition. Children will need to stay home if they are sick, or have any symptoms that are suspect of a communicable disease. Rochester Community Schools follows Oakland County Health Division guidance in determining exclusion criteria.

Please submit this plan for asthma. Inhalers can be ordered for office use or self-carry within this plan. A metered dosed inhaler (MDI), is preferred, but if a nebulizer treatment is required, please contact the school office, prior to school start.

Please note the following:
All medication must be in the original container or pharmacy prescription bottle labeled with the student's name, medication name, dosage and frequency to dispense instructions. NO EXPIRED MEDICATION will be administered. All forms are posted below. Click on the desired condition or subject and you will find the correct form and other resource information. In compliance with Section 504 law, all MAP forms include a notice of procedural safeguards.

Medication Administration
If medication is required, to be given at school, and is not part of a MAP order, such as Ritalin® for ADHD or Motrin® for arthritis, then the school medication administration authorization form is required for both prescription and over the counter (OTC) medication administration. One form per medication, is required, and needs to be signed by both the parent/guardian and the licensed prescriber. RCS will no longer accept parental permission alone, for (OTC) medications. Please see guidelines on the reverse side of the form for reference.

Diabetes Care
Please submit the school based health care plan and orders template that your child’s physician/licensed health care provider prefers. Most, if not all endocrinology clinics, do not have the ability to complete a school district’s template. Please see additional instructions listed in the diabetes MAP link, including instructions for routine care that may require a more involved plan for each individual student. The State of Michigan has a specialized team that is currently evaluating diabetes care across the state and will have updated regulations.
