Athlete Registration
Rochester Community Schools uses FinalForms to register middle and high school student-athletes, collect required signatures, and receive sports physical documentation.
Required Before Participation
A current sports physical is required
A valid MHSAA pre-participation physical must be completed on or after April 15 for the upcoming school year and uploaded to FinalForms before the student’s first practice or tryout.
Online Registration
What is FinalForms?
FinalForms is the secure external system RCS uses for athlete registration, required acknowledgements, digital signatures, and physical-form uploads.
First-time parent or guardian account setup

Open FinalForms
Select the Open FinalForms button on this page to visit the registration website.

Create a parent account
Under the Parent section, select New Account.

Enter your information
Enter your name, date of birth, and email address, then select Register.

Confirm your email address
Watch for an account confirmation email from FinalForms. Open the message and select Confirm Your Account. Check your spam or junk folder if the message does not arrive.

Create your password
Create a FinalForms password, then select Confirm Account.

Register your student-athlete
Select Register Student, add the student, choose the appropriate school and sport, and complete all required forms and signatures.

Upload the current physical
Upload the completed sports physical in FinalForms before the first day of practice or tryouts.
Returning family
Under the Parent section, select Login and use the email address connected to your existing FinalForms account. Then review and complete any forms required for the new season.

- Parent or guardian contact information
- Student-athlete name, birth date, grade, school, and sport
- Emergency contacts
- Health insurance information
- Completed current-year MHSAA physical form

MHSAA Physical Examination Clearance Form
This two-page form includes medical history, the provider’s examination and clearance, emergency information, insurance information, consent language, and required signatures.
Family completes: medical history, emergency information, insurance information, and consent sections.
Medical provider completes: examination findings, recommendations, and medical clearance.
