Affidavit for Placement of a Child Living in the Home of a Relative (3 Part)

Dental Assessment Form - For incoming kindergarten students

Shared Household Affidavit - For parents living with another family in the district when the home and residency documents are not in the parents' name

Birth Affidavit - Used in conjunction with other approved proof of birth when the birth certificate is not available. The birth certificate must be submitted to the Enrollment office within 30 days of enrollment.
