Dear Parents and Rochester Community School Staff,

New Medical Action Plans (MAPs) are required every school year for medical treatment at school. To be sure you are using the correct form, look for the current school year typed at the top of the MAP. All MAPs for next year should have 2013-2014 printed on the top.

Standardized MAPs are available for: Severe Allergy (life threatening), Asthma, Diabetes and Seizures. There is one General MAP for other conditions if needed. MAPS are intended to be guidelines and may be modified as appropriate by the attending physician.

Separate medication forms (Over-the-Counter and Prescription) are not needed for the four main MAPs (above). Medications for these conditions are to be ordered on page 2 of the MAP by the physician.

If medication is required as a part of a General MAP or as a stand alone order for something else, such as Ritalin for ADHD, then a separate prescription medication form is required.

Over-the-counter medications ordered by the parent should follow the guidelines offered on the reverse side of this form.

All forms are posted on the RCS web-site under the Parents and Community tab and then further under the title Health. By scrolling down to the bottom of this page you will see the labeled folders. Click on the desired condition and you will find the correct form and other information.

MAPs should be completed and signed by the parent/guardian (page 1) and ordering physician (page 2).

2013-14 MAPs are due August 15, 2013

If your child attends middle school or high school, please return your MAP to the main office of the school your child will be attending in 2013-2014. If your child attends elementary school, please return the MAP in a sealed envelope, clearly labeled MAP, to the reception desk at the Administration Building, 501 W. University, Rochester.
