All District employees involved in work-related injuries or illnesses REQUIRE the completion of Employee Injury Report which is available from your Supervisor or building secretary or below in the Employee Injury Packet. This form must be completed by your supervisor and returned to the Human Resources/Benefits Department within 7 days of the incident.

If medical attention is needed, authorization for treatment must be obtained from your Principal, Supervisor or Human Resources/ Benefits Department. You must then report to the authorized District Medical Clinic:
