The State of Michigan has passed a new law that hopes to increase awareness about concussion symptoms. This new law requires that all parents or guardians sign a concussion awareness form, which is below.

No student can participate in any scheduled athletic activity, including (but not limited to) sports physical education classes and recess unless this form is signed and returned to school.

The form below was sent home with students in athletic registration packets as well as on the first day of school. Please have this form completed before your child participates in any athletic activity. It may be returned to your child’s coach on the first day of practice, or to the school office.
